School of Choice Applications-All Grades and 5th/6th Grade Intent to Register Forms

School of Choice Applications

The following links will take you to forms that may be completed and submitted electronically (except for JK/TK forms). Each form is available in English or Spanish. If you have questions, please email Dr. Jason Messinger at jason.messinger@simivalleyusd.org or ask at your current school. You may also call the Educational Services Department at the District office at (805) 306-4500, Ext. 4200.

 

YOU WANT TO APPLY FOR A SCHOOL OF CHOICE (NOT YOUR NEIGHBORHOOD SCHOOL) FOR KINDERGARTEN THROUGH 6th GRADE IN 2017-2018: If you're interested in applying for School of Choice at an ELEMENTARY school (grades Kindergarten through 6th) that is NOT your neighborhood school, click ENGLISH or SPANISH. (Please note that Hollow Hills Elementary and Vista Elementary are Fundamental schools and are NOT Schools of Choice.)

 

YOU ARE THE PARENT OF A CURRENT 5th GRADE STUDENT IN ANY OF OUR SCHOOLS: If your child is currently in 5th grade in one of our schools, please click ENGLISH or SPANISH to complete the Intent to Register form to let us know if you will keep your child at his or her current elementary school, or if you intend to enroll him or her in either your neighborhood middle school or a School of Choice for the 2017-2018 school year. 

 

YOU ARE THE PARENT OF A 6th GRADE ELEMENTARY STUDENT IN OUR SCHOOLS: If your child is currently in 6th grade at an ELEMENTARY school, please click ENGLISH or SPANISH to let us know if you will enroll your child at your neighborhood middle school or in a School of Choice for the 2017-2018 school year.

 

FOR NEXT YEAR'S MIDDLE SCHOOL STUDENTS: If your child is entering 6th, 7th or 8th grade from a school outside of the Simi Valley Unified School District, or if your child is a current 6th or 7th grade student at a Simi Valley middle school and you wish to change schools for the 2017-2018 school year, either to your neighborhood school or to a School of Choice, please click ENGLISH or SPANISH.

 

FOR NEXT YEAR'S HIGH SCHOOL STUDENTS: If your child is entering high school or would like to change high schools and is applying for School of Choice (not your neighborhood high school) for the 2017-2018 school year, please click ENGLISH or SPANISH. (This applies to ONLY Royal High School and Simi Valley High School.)

 

FOR PROSPECTIVE SANTA SUSANA HIGH SCHOOL STUDENTS: If you're interested in having your child attend Santa Susana High School for the 2017-2018 school year, please click HERE for the application. Please note that Santa Susana High School is NOT a neighborhood high school and does not qualify for School of Choice. All students are selected through a lottery process and can apply in any grade.
